Incomplete forms cannot be considered.* Housing Policy: All single undergraduate students who do not return to their parent’s or legal guardian’s home each evening are required to live in University housing. Parent’s or legal guardian’s homes must be included in the following list of counties to commute: Boone, Hamilton, Hancock, Hendricks, Johnson, Madison, Marion, Morgan, or Shelby. Students who have attained their 21st birthday on or before August 31 may request a policy waiver through the Office of Housing and Residence Life. * denotes required fields

Acceptance and Verification The student and parent/legal guardian understand and accept the student has been exempted within the Housing Regulation in order to reside only at the indicated address. Further, should a change in the registered address be made by the student, consultation with the Office of Housing & Residence Life is necessary to determine eligibility for continuing exemption from the housing regulation. The signatures below verifies that all information provided is accurate and complete and that the furnishing of false information may result in cancellation of enrollment, payment for unpaid housing fees, or in disciplinary action against the student pursuant to the Code of Student Rights and Responsibilities.
