Student Health Record and Immunization Record
The Student Health Record and Immunization Record are required to be completed by all students. It must be submitted to the Director of Health Services, Marian University, 3200 Cold Spring Road, Indianapolis, IN 46222 before registration.

Consent for Release of Confidential Information
The Marian University Student Health Center staff cannot talk to a student’s parents or significant other until this form is signed by the student and on file in the Student Health Center.  

Consent for Release of Medical and Immunization Records
The Marian University Student Health Center staff cannot release medical or immunization records until this form is signed by the student and on file in the Student Health Center.
